How it works:

  1. Fill out the information in the form below.
  2. Attach a photo of your existing dental treatment plan with procedure codes.
  3. We will email you a treatment estimate with our fees.

Please note that this portal is for patients that have received a recent estimate from another dental clinic.

Please be aware that we will provide our fees for the procedures that you have been quoted for. However, your oral health condition will need to be assessed in our office prior to beginning treatment as our clinicians need to confirm your treatment plan.
